room A2-469 and placed on the Accessioning Table. Only Specimens will be
accessioned and processed. Placentas delivered for disposal only should
be marked as “Non-Specimens” and not accessioned.

in the upper right corner. The remaining labels are clipped to the

One label should be affixed to each specimen container

Only Specimens will be accessioned and processed.

Ordering: Surgical Pathology Protocol Form (sp-1) Must be
completed
Ordering – Tests are not to be ordered through the computer.
Collection:
Acceptance – No specimen is accepted which is not accompanied by a properly completed Surgical Pathology Protocol Form (SP-1). It is necessary to write clearly on a Form #SP-1. One form per procedure is required regardless of the number of specimens submitted. Also, all specimen containers submitted must have a completed label affixed to it.
NOTE: Questions about how to submit specimens or what diagnostic procedures are available should be directed to one of the pathologists in Surgical Pathology.
