Transport and Delivery:

 

1.       Mon-Fri, 7:00am to 6:00pm – All Specimens are to be delivered to room A2-469 and placed on the Accessioning Table.  Only Specimens will be accessioned and processed.  Placentas delivered for disposal only should be marked as “Non-Specimens” and not accessioned.

Patient’s Medical History:

 

·          Nature of tissue submitted (describe and illustrate).

 

·          Clinical diagnosis.

 

·          Brief summary of medical condition with lab test results.

 

·          Medication patient is taking (current & previous).

 

·          Applicable family medical history.

 

·          Knowledge of previous pathology reports (here & elsewhere)

 

·          Date of current transplant & origin of organ/tissue.

 

Submitting Physician’s Information:

 

·          Physician’s Full Name.

 

·          Physician’s Telephone Number or beeper number.

 

·          Hospital (if outside source).

 

·          A complete Specimen Label consists of.

 

·          Patient’s Full Name.

 

·          Patient’s Medical Record Number.

 

·          Patient’s Admission Number.

 

·          Date tissue was sent to Surgical Pathology.

 

·          Exact nature of tissue in the container.

 

·          Clinical diagnosis.

·          Placentas delivered for disposal only marked as “Non Specimens” and not accessioned.

 

Computer Labels:  Each case should have large computer printed labels containing:

 

·          Patient’s Name.

 

·          Medical Record Number.

 

·          Patient (Admission) Number.

 

·          Nursing Station or Clinic.

 

·          Use of computerized labels:

 

1.       One label is affixed to the Surgical Pathology Protocol Form in the upper right corner.  The remaining labels are clipped to the form.

2.       One label should be affixed to each specimen container submitted.

 

 

 

 

 

1.       Mon-Fri, 6:00pm to 7:00 am – All Specimens must be placed in refrigerator beside room A2-423.  Only Specimens will be accessioned and processed.

 

1.       Sat-Sun, & Holidays – All Specimens must be placed in refrigerator beside room A2-423.

Ordering:  Surgical Pathology Protocol Form (sp-1) Must be completed

 

Ordering – Tests are not to be ordered through the computer.

 

Collection:

 

Acceptance – No specimen is accepted which is not accompanied by a properly completed Surgical Pathology Protocol Form (SP-1).  It is necessary to write clearly on a Form #SP-1.  One form per procedure is required regardless of the number of specimens submitted.  Also, all specimen containers submitted must have a completed label affixed to it.

 

NOTE: Questions about how to submit specimens or what diagnostic procedures are available should be directed to one of the pathologists in Surgical Pathology.
